Form Questions

When you are ready to add fields to your form begin by clicking the Add Questions/Manage Questions button. There are nine types of form fields. Click the green plus sign icon to begin adding fields to your form. Choose the type of field and enter the text of the question into the text field. To remove a field, click Delete This Field.

Additional options are available for each individual field. Click Show Options to display these. By default all fields are set as required but you can deselect this checkbox for optional fields. You can also enter a Description for each which will appear in light gray text directly below the field. Additional Options will depend on the field type.

  • Line of text fields allow users to enter in a small line of text. Using the provided drop down menu, this type of field can be set to automatically pull the current user's First, Last, or Full name (only for Course or Protected forms). Additional options include setting a minimum and maximum amount of characters.
  • Paragraph fields allow users to enter multiple lines of text. Additional options include setting a minimum and maximum amount of characters.
  • Number fields allow users to answer questions that require numbers for answers. Additional options include setting minimum and maximum number values.
  • Email fields allow users to enter their email address. Additional options allow you to check a box so that a confirmation email is sent to the user submitting the form.
  • Date fields allow users to select a date. It does not need to be the current date. Keep in mind that this does not reflect the submission date, which is automatically generated when the form is submitted.
  • File fields provide a location for users to upload and attach a file to their form submission.
  • Checkbox group fields allow users to check multiple options from a list. By default there are three answer options available. Click Add Another Option to add more check boxes to your list. Additional options allow you to add a free text option for custom responses.
  • Drop down list fields allow users to select one answer from a drop down list of options. By default there are three answer options available. Click Add Another Option to add more.
  • Radio group fields allow users to select one answer from a list of options. By default there are three answer options available. Click Add Another Option to add more.
